29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): Vacuum Pipe Still 2 (VPS-2). The employer did not ensure that the energy isolation device (TV139 with associated valves & piping) in the High Pressure Boiler Feed Water Line was closed to prevent backflow/leakage of high pressure water into the 600 psi steam line between 83H101 (Charge Tower) and the 83D101 (Steam Desuperheater). This exposed employees to the sudden and un-expected release of steam and/or hot water at approx 240 deg F.

29 CFR 1910.147(d)(5)(ii): Where there was a possibility of re-accumulation of stored energy to a hazardous level, verification of isolation was not continued until the servicing or maintenance was completed, or until the possibility of such accumulation no longer existed: Vacuum Pipe Still (VPS-2). The employer did not continually verify that the 600 psi steam line between 83H101 (Atmos. Tower Charge Heater) and 83D103 (Desuperheater) remained isolated and the re-accumulation of steam and/or hot water was not occuring between the time that the unit was shut down and the blinding of the steam lines was accomplished. This exposed employees to the sudden and un-expected release of steam and/or hot water at approx 240 deg F.g
