Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish to each of his employees a place of employment which was free from
recognized hazards
that were causing or likely to cause death or serious phyical harm to
employees in that
employees were exposed to the hazard of the sudden and unexpected
movements of the vessel
due to obstructions in the path of travel:
On or about 12/06/08, during the transportation of employees via crew boat
from the barge rig
Liberty in the Delta Farm Lake, approximately 25 minutes south of Larose
LA, to the employers
shore facility, employees were not protected from sudden, unexpected,
unseen and potentially
violent movements of the vessel due to obstructions in the path of travel
at night, such as, but
not limited to, barges placed in the waterway.
Among other methods, some feasible and acceptable method(s) to correct
this hazard would be
to:
1.  Install floodlights on the crew boats for use at night in remote
locations to illuminate the
path of travel and identify obstructions.
2.  Require all employees being transported by crew boat to remain seated
at all times when
traveling at night.
3.  Install improved navigational equipment on crew boats that would be
capable of
identifying obstructions in the path of travel, and to provide training to
all boat skippers to
properly operate the navigational equipment.
