which were free from recognized hazards that were causing or likely to
cause death or serious
physical harm to employees.
In that employees were exposed to cuts and falls of 10 feet, while walking
on scrap metal in
roll-off containers during placing and removal of the tarp on the
containers, when the automatic
tarp system malfunctions. One feasible method of correcting the hazard
would be for employees
to use of portable ladders to work from, when adjusting the tarp over
protruding metal.
