1910.134(h)(2)(i): All respirators shall be stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals, and they shall be packed or stored to prevent deformation of the facepiece and exhalation valve: On or about 4-11-08, the employer did not ensure that respirators (half masks) used by employees working in a permit required confined space (Cat Cracker) while engaged in the removal of catalyst and/or other activities were stored adequately. The respirators were not properly enclosed and sealed and were exposed to particulates such as, but not limited to, silica.

Testing for oxygen content, for flammable gases and vapors, and for potential toxic air contaminants was not documented. 29 CFR 1910.334(a)(2)(i): Portable cord and plugconnected electric equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): On or about 4-11-08, the employer did not ensure that a red SJTW 16 AWG extension cord connected to a DeWALT grinder was visually inspected before use. The cord's outer and inner insulation was damaged exposing the bare conductors. This condition exposed employees to the hazards associated with electrical shock, sparks, and arc.
