Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Forming Department, South End - the foot treadle on the Feco Roto Die hydraulic press brake, Model Number 15, Serial Number 98026, was not guarded from unattended operation of the press brake. (b) Forming Department, North End - the foot treadle on the Feco Roto Die hydraulic press brake, Model Number 15, Serial Number 80718, was not guarded from unattended operation of the press brake. Among other methods, one feasible and acceptable method to correct this hazard includes but is not limited to the following: In accordance with ANSI B11.3 - 2002 - American National Standard Institute for Machined Tools - Safety Requirements for Power Press Brakes; Section 6.10.3 "When provided, a foot treadle of the type that serves as an operator control shall be equipped with a means to prevent unattended machine actuation. No abatement certification or documentation is required for this item.
