the controlling employer did not ensure that its sub-contractor Dan's
Welding
& Fabrication LLC  instructed and/or enforced the utilization of the seat
belts
by operator(s) of the forklift(s).

correct
this
hazard are by complying with the manufacturer's operation guide regarding
use of the
manufacturer supplied seatbelt and/or QNWI/ITSDF B56.1-2005 (Reaffirmation
of ASME
B56.1-2004), Section 7.41, Operator Restraint Systems.
Note(2): Abatement certification and documentation ARE required for this
item.
