that were causing or were likely to cause death or serious physical harm

that, employees were exposed to crushing hazards by operating a powered
industrial truck
without wearing a seatbelt.
a)Storage Yard:  On or about May 27, 2008 and at times prior thereto,
employees were exposed to crushing hazards by operating an Allis-Chalmers
forklift without using a seat belt while transporting granite slabs and
sheet
metal.
Abatement note:  Among other methods, one feasible and acceptable means of
abatement to correct this hazard would be to install and require the use
of seatbelts on
the forklift as prescribed in ANSI B56.1a-1989.
Note:  Abatement certification and supporting documentation are required
for this item.
