operating a forklift without using the seatbelt:
In the shop: On or about June 16, 2009, and at times prior thereto,
employees operating the
Hyster H35XL forklift did not use the seatbelt nor were they required to
do so.
Note:  Abatement certification required for this item.
Note: Among other methods one feasible and acceptable means of abatement
to correct this hazard would be to
establish a work rule that seatbelts be worn when operating the powered
industrial truck and ensure the work
rule is enforced by conducting periodic inspections of the operators.s
