hazard of being struck by pieces or
sections of a failing composite fiber cutting disc:
a)North side of excavation in open area; On or about 4/7/09 and at times
prior thereto,  an employee was
utilizing the side of a composite fiber cutting disc on a gas powered
cut-off saw to bevel the end of a
PVC pipe.
Note 1: Abatement certification IS required for this item.
Note 2: A feasible means of abatement includes but is not limited to
following ANSI B175.4 - 2000, for
'Portable, Handheld, Internal-Combustion-Engine-Driven Cut-Off Machines
-Safety Requirements", the
Manufacturer's Operator Guide and/or the Cut-off Disc's Manufacturer's
warnings against using the side of
the disc for grinding. Using a cut-off saw/angle grinder beveling
attachment
or an angle grinder with a flap type
sanding/grinding wheel are several of many options for beveling the PVC
pipe.ing
