Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were likely to cause death or serious physical harm to the employees in that employees were exposed to struck by/caught between hazards from roof truss collapse during installation: a)Jobsite: On April 23, 2009, and times prior thereto, employees performing framing activities were exposed to approximately 20 trusses with 26 foot, 6 inch spans, without top cord diagonal bracing installed. b)Jobsite: On April 23, 2009, and times prior thereto, employees performing framing activities were exposed to 26 foot 6 inch truss spans that were installed without sufficient lateral bracing. NOTE 1) Feasible and acceptable methods of abatement to correct this hazard would be to follow either the manufacturer's recommendations on roof truss erection or the Building Component Safety Information 2008 edition, Guide to Good Practice for Handling, Installing, Restraining & Bracing of Metal Plate Connected Wood Trusses. Note 2) Abatement certification is required for this item.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were likely to cause death or serious physical harm to the employees in that employees were exposed to struck by/caught between hazards from roof truss damage and failure: a)Jobsite: On April 23, 2009, and times prior thereto, the employer used a single pick point to hoist and install 26 foot 6 inch truss spans. NOTE 1) Feasible and acceptable methods of abatement to correct this hazard would be to follow the manufacturer's recommendations on roof truss erection and the 2008 edition of Building Component Safety Information, Guide to Good Practice for Handling, Installing, Restraining & Bracing of Metal Plate Connected Wood Trusses. This states using a single pick-point at the peak can damage the truss. For single trusses up to 30 feet use a minimum of two pick-points near top chord joints spaced up to one half the truss length apart. Note 2)Abatement certification is required for this item.
