Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to dust explosion, deflagration, or other fire hazards because methods were not utilized to ensure proper storage and prevent ignition of combustible wood dusts during storage: (a) Sauder Manufacturing Company, Archbold, Ohio: On or about November 12, 2008, the wood dust storage silo located outside of the north exterior wall was not equipped with deflagration relief venting. The building and the employees working therein were exposed to a deflagration explosion hazard from wood dust in the storage silo. Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard is to equip the storage silo with deflagration relief venting in accordance with the National Fire Protection Association (NFPA) 664-2007 Standard for the Prevention of Fires and Explosions in Wood Processing and Woodworking Facilities section 8.10.3.2. Abatement Schedule Step 1 -A written detailed plan of abatement shall be submitted to the Area Director outlining a schedule for the implementation of engineering controls, instrumentation, and equipment to protect employees from fire and deflagration hazards related to combustible dust as referenced in this citation. This plan shall include, at a minimum, target dates for the following actions which must be consistent with the abatement dates required by this citation: (1) Evaluation of engineering control options; (2) Selection of optimum control methods and completion of design; and (3) Procurement, installation and operation of selected control measures; All proposed control measures shall be approved for each particular use by a person competent in fire and deflagration control of combustible dusts. At the conclusion of the 60-day period, the detailed plan for abatement shall be submitted to the Area Director. Step 2 -Abatement shall have been completed by the implementation of feasible engineering controls upon verification of their effectiveness in achieving compliance. Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to dust explosion, deflagration, or other fire hazards because methods were not utilized to ensure proper collection and prevent ignition of combustible of wood dust during collection operations: (a) Sauder Manufacturing Company, Archbold, Ohio: On or about November 12, 2008, Baghouse No. 1 and Baghouse No. 2, located outside of the north exterior wall, had four explosion vents directed over the processing facility. The venting was not directed to a safe location, away from the occupied processing facility. Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard is to direct the explosion vents away from the structure in accordance with the National Fire Protection Association (NFPA) 68-2007 Standard on Explosion Protection by Deflagration Venting Sections 6.6.1 and 6.6.2. (b) Sauder Manufacturing Company, Archbold, Ohio: On or about November 12, 2008, employees were exposed to fire and a deflagration hazard from recycling air from the Exhaust system/Baghouse No. 1 and Exhaust system/Baghouse No. 2 into the facility. No means was provided to prevent the fire and deflagration hazard entering the occupied facility through the recycled air system. Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard is to interlink the existing spark detection system with high-speed abort gates on the two exhaust air duct conveying recycled air back into the building. The abort gates are to divert any burning material to the atmosphere before it can enter the facility in accordance with the National Fire Protection Association (NFPA) 664-2007 Standard for the Prevention of Fires and Explosions in Wood Processing and Woodworking Facilities section 8.2.2.6.2(1) through (3). Abatement Schedule Step 1 -A written detailed plan of abatement shall be submitted to the Area Director outlining a schedule for the implementation of engineering controls, instrumentation, and equipment to protect employees from fire and deflagration hazards related to combustible dust as referenced in this citation. This plan shall include, at a minimum, target dates for the following actions which must be consistent with the abatement dates required by this citation: (1) Evaluation of engineering control options; (2) Selection of optimum control methods and completion of design;and (3) Procurement, installation and operation of selected control measures; All proposed control measures shall be approved for each particular use by a person competent in fire and deflagration control of combustible dusts. Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to dust explosion, deflagration, or other fire hazards because methods were not utilized to ensure proper trasnport and prevent ignition of combustible wood dusts during the transport operations: (a) Sauder Manufacturing Company, Archbold, Ohio: On or about November 12, 2008, a vertical and diagonal screw conveyor connects Baghouse No. 1 and Baghouse No. 2 to the storage silo. No means was provided to prevent the propagation of a deflagration to the storage silo through the screw conveyor. Employees were exposed to deflagration hazard. Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard is to install a choke in each conveyor to prevent the propagation of a deflagration between the baghouses and the storage silo in accordance with the National Fire Protection Association (NFPA) 664-2007 Standard for the Prevention of Fires and Explosions in Wood Processing and Woodworking Facilities, section 8.2.4.1 and 8.10.3.3. Abatement Schedule Step 1 -A written detailed plan of abatement shall be submitted to the Area Director outlining a schedule for the implementation of engineering controls, instrumentation, and equipment to protect employees from fire and deflagration hazards related to combustible dust as referenced in this citation. This plan shall include, at a minimum, target dates for the following actions which must be consistent with the abatement dates required by this citation: (1) Evaluation of engineering control options; (2) Selection of optimum control methods and completion of design; and (3) Procurement, installation and operation of selected control measures; All proposed control measures shall be approved for each particular use by a person competent in fire and deflagration control of combustible dusts.
