Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) On or about 11/29/07 in the Red Building; during the defrosting of Room 9 evaporator piping containing ammonia located on the ceiling of Room 11 were struck and hit with wooden 2x4's and other pieces of woods to loosen frost and ice build up subjecting piping to damage and or leaks. One feasible and acceptable abatement methods to correct this condition would be; eliminate the overhead piping as a cooling/refrigeration source and install updated refrigeration system, allow frost and ice to melt naturally, use external heat sources to increase temperature of Room 11. Reference ANSI/ASHRAE 15-1994 Safety Code for Mechanical Refrigeration - Section 7 - Restrictions on Refrigerant Use, 7.2 - General Restrictions- Safeguards. ABATEMENT DOCUMENTATION REQUIRED

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) On or about 11/29/07 at the Red Building; a powered industrial truck weighing approximately six thousand nine hundred and fifty pounds was loaded onto the freight elevator with a capacity of four thousand pounds exceeding the capacity. One feasible and acceptable abatement methods to correct this condition would be; conspicuously position and permanently secure to the inside of the car enclosure signs indicating the prohibition of overloading the freight elevator, train Arctic Glacier employees and contractors on the capacity of the freight elevator and the probation of overloading freight elevator. Reference: ASME A17.1 - 2000 Safety Code for Elevators and Escalators Part 2.16.5 & 2.16.6 ABATEMENT DOCUMENTATION REQUIRED
