North Carolina General Statute 95-129(1) of the Occupational Safety and
Health Act of North Carolina:  The
employer did not furnish to each of his employees  conditions of
employment and a place of employment free
from recognized hazards that were causing or likely to cause death or
serious physical harm to employees, in
that employees were exposed to:
a)Pusher furnace, Super Allcase Line, and Standard Allcase Line - the
integral quence
furnaces that were used for hardening and tempering processes were
supplied oxygen
deficient atmospheres via endo-equivalent systems.  The endo-equivalent
systems
created oxygen deficient atmospheres primarily composed of carbon monoxide,
methane, and carbon dioxide, which was inside of the furnaces. The
percentage of
carbon monoxide was 12, or 12,000 parts per million.  There were no carbon
monoxide alarms in the areas around the integral quence furnaces.
NOTE:  Among other methods of abatement, one feasible and acceptable
method
to correct
this hazard is to install carbon monoxide alarms in the production areas
where the integral
quence furnaces were operating.

North Carolina General Statute 95-129(1) of the Occupational Safety and
Health Act of North Carolina:  The
employer did not furnish to each of his employees conditions of employment
and a place of employment free
from recognized hazards that were causing or likely to cause death or
serious physical harm to employees, in
that employees were exposed to:
a)Pusher furnace area - the one ton jib crane used to lift racks of metals
from the pusher
furnace conveyer was equipped with an under-the-hook lifting device that
had not been
load rated, and was not marked with a load rating.  The under-the-hook
lifting device
consisted of a fabricated steel bar with a chain, and lifted approximately
700 pounds,
not more than 4 feet above the floor.

Construction:
The
rated load of the lifting device shall be marked on the main structure
where it is visible.
NOTE:  Among other methods of abatement, one feasible and acceptable
method to correct
this hazard is to complete load testing on the under-the-hook lifting
device, and label the
under-the-hook lifting device with the load capacity.-hook
