Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that ere causing or likely to cause death or serious physical harm to employees in that employees are exposed to crushing/amputation hazard from unintended activation: a.Sheet Metal Shop - The foot pedal/treadle on the Roto-Die hydraulic press brake (serial #8052) was not guarded, as determined on 04/08/2008. ABATEMENT NOTE: Among other methods one feasible and acceptable abatement method to correct this hazard is to guard the foot pedal/treadle in accordance with ANSI B11.3-2002, "Safety Requirements for Power Press Brakes."
