Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to crushing injuries and amputations while
conducting operations
such as placing paper in the work area of the Challenge drill:
(a)Bindery Department: The Challenge drill had a foot operated tripping
control
that was not protected by a guard, on or about March 9, 2010.
(b)Bindery Department: The Wohlenberger Type 115, S/N 3416-008, paper
shear, had a foot operated tripping control that was not protected by a
guard,
on or about March 9, 2010.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to install a guard on the foot pedal so as to prevent operation
from falling or
moving objects, or from unintended operation by accidental stepping onto
the foot
control.
