Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were likely to cause death or serious physical harm to the employees
in that employees
were exposed to fire hazards:
(a)NEX 512, Production Area:  On January 22, 2009 and times prior to,
employees were exposed to a fire hazard while charging the fluid bed with
powdered ingredients used in the manufacturing of pharmaceuticals.  The
powdered ingredients were a combustible dust.  The drop tube used for
charging was not equipped with a device to prevent fire from entering into
the
work area from the fluid bed.
NOTE: Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to follow accepted safe practices such as but not limited to;
(a) installation of an
isolation valve between the fluid bed and operator opening of the drop
tube, (b) follow the
guidelines set forth in NFPA 654, Standard for the Prevention of Fire and
Dust
Explosions
from the Manufacturing, Processing, and Handling of Combustible
Particulate Solids.
