Title 40.1-51.1.(a); Code of Virginia: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: (a)the Caterpillar Forklift, Model # M50D, was not used in accordance with ANSI Standard B56.1 1c-1991, Section 5.2.24(a) which does not permit a forklift to be used to elevate personnel on an unsecured platform. PolyFab Display Company permitted its employees to elevate personnel using a plastic crate lifted by the tines of a forklift; the plastic crate was not secured to the forklift when it was being used as a manlift at 14892 Persistence Drive, Woodbridge, VA. Employee(s) exposed to a fall hazard. b) The owner or employer did not require the plastic crate to be secured to the forklift while being used to elevate employees. Employees were exposed to a fall hazard due to a platform being applied to the forklift and not securing the platform to the forklift. Note: One feasible and acceptable means to correct this hazard includes training employees to follow the recommendations in ANSI Standard B56.1 1c-1991, Section 5.2.24(a). Another feasible and acceptable means to correct this hazard would be to end the practice of using the forklift and plastic crate as a manlift.
