Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to caught-in hazard due to a change to manual handling of
pipe joints:
On or about January 28, 2008, and at other times prior thereto, at this
well site,
the employer did not review steps/cautions/practices for inexperienced
employees
(derrickman) handling pipe joints where pipe connections, elevators,
collars, and
stand were handled manually for a temporary duration.
Among others, one feasible and acceptable abatement method to address this
hazard would include implementing the developed Job Safety Analysis (JSA)
process when routinely used equipment (primary air hoist or Willie Winch)
has
broken down and practices must be altered and re-evaluated.  This abatement
method would adhere to the employer's written safety and health program
and
the
International Association of Drilling Contractors' "Accident Prevention
Reference
Guide" (1998), Sections 1.4, 3.7.
