were causing or likely to cause death or serious physical harm to
employees, in that, employees
were exposed to potential crushing injuries in the event of a fork truck
tipover.  Fork truck
operators were not required to use seat belts that were provided on the
fork trucks:
Markel Industries:  An employee operating a Mitsubishi forklift, serial
number AF81B50493,
was not wearing the provided seatbelt.
Among other methods, one feasible method to correct this hazard is to
require and enforce the
use of seatbelts when operating the forklift.
