General-duty citation text

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees, in that employees
were exposed to crushing and/or fall hazards:
In the following locations the structural strength of the building had
been compromised
(weakened) by extensive rust and corrosion as well as by physical damage.
A.The roof support structure over the #1 and #2 savealls was rusted and
corroded.  A roof
support beam had failed causing a partial roof collapse.
B.The support structure for the wire tray mezzanine was rusted and
corroded.
C.The support structure for the catwalks at the felt rolls in the basement
had extensive
corrosion.
D.The building support structure at the wet end of the paper machine was
rusted and
corroded. Building I beam columns B3 through B8 had severe corrosion.
Cantilever arms
to support the paper machine were severely corroded.
E.The concrete floor behind the panel room (above the old AD-PD process)
and the
concrete floor in the beater room (near the cyclone cleaners) were
structurally unsound.
The support structure in the basement below these two areas had support
beams
that had
failed.
F.The support legs for the vacuum separator tanks at the wet end of the
paper machine
were severely corroded.
G.2x6 wooden beams at the ceiling over the wet end of the paper machine
were sagging
H.A portion of the sheet metal wall at the #3 fan at the wet end of the
paper machine was
hanging loose.  The fan ductwork was being held up with a piece of channel
iron and
threaded rods.
I.The bases of several support columns in the warehouse had been damaged
and bent out
of shape.
J.The metal door frame at the overhead door in the basement bullpen was
damaged.
K.A section of the paper machine operating room floor slab "delaminated."
Pieces of loose
concrete fell to the basement near the main line shaft motor.
Among other methods the following are feasible and acceptable means of
abating the hazards:
A.Under the direction of a qualified structural engineer immediately
schedule repairs to the
roof structure over the #1 and #2 savealls, the wire tray mezzanine
structure, and the
catwalks at the felt rolls in the basement.
B.Under the direction of a qualified structural engineer schedule repairs
to the building
support structure at the wet end of the paper machine, the concrete floor
behind the panel
room and the concrete floor in the beater room as soon as possible.
C.Under the direction of a qualified structural engineer schedule repairs
to the support legs
for the vacuum separators, the wooden beams at the ceiling, the number 3
fan support
structure and adjacent wall, the damaged warehouse columns and the
basement
bullpen
door frame in a timely manner.
D.Immediately establish a monitoring program to ensure the stability of
structural members
in the roof structure over the #1 and #2 savealls, the wire tray
mezzanine, catwalks at
the basement felt rolls, building support structure and cantilevers at the
wet end of the
machine, the concrete floor behind the panel room and the concrete floor
in the beater
room.
E.Retain the services of a structural engineer to perform a comprehensive
structural survey
of the integrity of the entire plant.  Include in the survey an inspection
of all concrete
slabs in the plant to determine if any additional areas have "delaminated"
or deteriorated.
Prioritize a list of any additional deficiencies found and schedule
corrections in a timely
