Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential fire and explosion hazards: Grinding/Polishing Department: Combustible aluminum and titanium dust generated during finishing operations was not collected into a adequately designed dust collection system: 1.Finishing of aluminum and titanium parts was done at work benches that were not provided with a dust collection system or hood. Fugitive dust was allowed to migrate into the work area and settle on horizontal surfaces such as the floor, work bench and overhead light fixtures. NFPA 484, Section 6.3.2.1. 2.All grinding and polishing was collected into the same dust collection system. NFPA 484, Section 6.3.2.4 and 10.4.4.4. 3.The dust collector and associated ductwork was used for aluminum, titanium and Inconel grinding and polishing. Grinding of titanium and inconel introduced sparks into the dust collection system. NFPA 484, Section 6.3.2.6. 4.The dust collector and machines were not posted that they were for use with combustible metals and different type of metals were processed into the same dust collection system. NFPA 484, Section 6.3.2.6.1 and 10.4.4.5. 5.The sump of the RotoClone dust collector was not vented at all times. NFPA 484, Section 6.3.4.6 and 10.4.7.6.1. 6.The electrical supply for the grinding/polishing machinery was completely independent of the dust collection systems electrical supply and the machines could be run with the dust collector off or malfunctioning. NFPA 484, Sections 6.3.4.7.1, 6.3.4.7.2, 10.4.4.6.1 and 10.4.4.6.2. 7.Metal (steel) dust pans and brushes were used to clean work areas. A metal handled and bristled brush was provided for cleaning out the ductwork. NFPA 484, Sections 6.4.2.2 and 6.5.7.7.3. 8.A Hafco model HV-55-1510V portable air driven vacuum cleaner was provided to clean fugitive dust that was not approved or listed for use with combustible metal dust, was provided with a plastic hose and steel attachment parts. NFPA 484, Section 6.4.3. Among other abatement methods, one feasible and useful method to correct the hazard is to: 1.Review National Fire Protection Association (NFPA) Standard 484 Standard for Combustible Metals regarding the proper design, installation of use and dust collection systems for use with operations that generate combustible dust. 2.Ensure that all dust generating operations are segregated and provided with suitable dust collection equipment. Specific Abatement Documentation is Required
