General-duty citation text
40.1-51.1(a) of the Code of Virginia: The employer, Liphart Steel Company, Inc., did not furnish a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were operating chain hoists whose hooks were not equipped with safety latches as required by ANSI B30.16-1987 (Hoists). At their Verona Va., facility In the Paint Shop[ and the Filter-Welder work station, two 5 Ton Robbins & Myers Electric Powered Chain Hoists are used by employees to lift steel beams ranging in weight from 100 lbs to 4,000 lbs. Three hooks for these hoists were found to be missing the safety latches. One feasible and acceptable method, among other methods, to correct this hazard is to install latch type hooks such as that called for by ANSI B30.16-1987, American National Standard Safety Code for Hoists.
