Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to amputation hazard:
a)  Establishment:
Employer did not ensure plastic extrusion machines were periodically
inspected for malfunctions
including but not limited to fully operating interlocks.
Violation observed on or about 3/7/2008.
Among other methods, feasible and acceptable abatement methods to correct
this hazard would
be to ensure employees required to operate and maintain the plastic
extrusion machinery conduct
a regular and period inspection of the equipment, in accordance with:
1) The manufacturer's recommendations, and
2) ANSI/SPI B151.7-1996, American National Standard for Plastics Machinery
- Plastics
Extrusion Machines - Requirements for the Manufacture, Care and Use,
Section 4.3 Inspection
and Maintenance, which states "It shall be the responsibility of the
employer to establish and
follow a program of periodic and regular inspections of the extruder to
ensure that all safeguards
are in proper operating condition and are properly adjusted".
Note:  Abatement certification and documentation are required for this
item.
