Section 5(a)1 of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to fire and explosion hazards:
Employees working in the blending room were exposed to fire and explosion
hazards for
the following conditions:
a)Blending Room Dust Collector:  The dust collector was located inside the
blending room.
b)Blending Room Dust Collector:  The exhaust from the dust collector was
not
discharged outside.
c)Blending Room Dust Collector:  The dust collector and duct work were not
bonded and grounded.
d)Blending Room Dust Collector: The dust collector was not designed with
explosion protection.
e)Blending Room Dust Collector and Pneumatic Conveyor System:  Isolation
devices were not provided between pieces of equipment connected by duct
work.
Among other feasible and acceptable methods to correct these hazards are
to follow NFPA 654
Prevention of Fire and Dust Explosions from the Manufacturing, Processing,
and Handling of
Combustible Particulate Solids, 2006 Edition including:
a)Move the dust collector outside as per Section 7.13.1.1.1.  "Where and
explosion
hazard exists, air-material separators shall be located outside of
buildings."
b)Ensure the exhaust is vented outside as per Section 7.13.1.7.1.
"Exhaust air
from the final air-material separator shall be discharged outside to a
restricted
area and away from air intakes."
c)Ground the dust collector and bond all connecting ductwork to the dust
collector
as per Section 9.3.1.3.  "Bonding and grounding with a resistance of less
than 1.0
times ten to the 6th ohms to ground shall be provided for conductive
components."
d)Design and install a dust collector rated for combustible particulate as
per Section
7.1.2.1.  "The design of explosion protection for equipment shall
incorporate one
or more of the following methods of protection:"
1)Oxidant concentration reduction in accordance with NFPA 69, Standard
on Explosion Prevention Systems
a)Where oxygen monitoring is used, it shall be installed in
accordance with ISA 84.00.01, Functional Safety: Application of
Safety Instrumented Systems for the Process Industry Sector.
b)Where the chemical properties of the material being conveyed
require a minimum concentration of oxygen to control
pyrophoricity, that level of concentration shall be maintained.
2)Deflagration Venting
3)Deflagration pressure containment in accordance with NFPA 69, Standard
on Explosion Prevention Systems
4)Deflagration suppression systems in accordance with NFPA 69, Standard
on Explosion Prevention Systems
5)Dilution with a noncombustible dust to render the mixture noncombustible
6)Deflagration venting through a listed dust retention and flame-arresting
device
e)Install devices to prevent the propagation of a fire or explosion as per
Section
7.1.4.1.  "Where an explosion hazard exists, isolation devices shall be
provided
to prevent deflagration propagation between pieces of equipment connected
by
ductwork."
Section 7.1.4.2, Isolation devices shall include, but shall not be limited
to the
following:
1)Chokes
2)Rotary valves
3)Automatic fast-acting valve systems in accordance with NFPA 69,
Standard on Explosion Prevention Systems
4)Flame front diverters in accordance with NFPA 69, Standard on Explosion
Prevention Systems
5)Chemical isolation systems in accordance with NFPA 69, Standard on
Explosion Prevention Systems
29 CFR 1903.19(d)(1) requires certification and documentation that the
abatement of the
above violation is completed.
