Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injuries and amputation hazards while operating and during periodic cleaning and maintenance of the ANETS dough roller: a) 479 Chevron Street, The Landings, Dover AFB, DE: On or about December 15, 2010 and at times prior to, employees were exposed to crushing injuries and amputation hazards while cleaning the ANETS dough roller, Model Number MDR-6C located in the pizza making area. Employees are required to use the equipment on a daily basis. Employees are not trained and/or provided proper instructions in the safe operation and use of equipment in accordance with the ANETS dough roller Operation/Safety Manual. ONE OR MORE FEASIBLE AND ACCEPTABLE ABATEMENT METHODS, AMONG OTHERS, TO ABATE THIS HAZARD IS TO PROVIDE TRAINING AND SAFETY INSTRUCTIONS TO EMPLOYEES IN ACCORDANCE WITH ANETS DOUGH ROLLER OPERATION SAFETY MANUAL AND ANSI Z50.1 AND ANSI Z50.1-a, BAKING EQUIPMENT/SAFETY REQUIREMENTS. This would include the following: 1. UTILIZE ALL SAFETY INSTRUCTIONS and KEEP THEM READILY AVAILABLE FOR MACHINE OPERATORS. 2. MAKE CERTAIN ALL OPERATORS ARE PROPERLY TRAINED, KNOW HOW TO SAFELY AND CORRECTLY OPERATE THE UNIT AND ARE PROPERLY SUPERVISED. 3. ALLOW OPERATION OF THE UNIT ONLY WITH PARTS (INCLUDING SAFETY EQUIPMENT) IN THEIR CORRECT POSITION. 4. INSPECT AND MAINTAIN THE UNIT ON A REGULAR BASIS AND PERFORM ALL MAINTENANCE AS REQUIRED. 5. INSURE THAT ALL WARNING LABELS ON THE UNIT ARE CLEAN AND READABLE. 6. ASSURE THESE PROCEDURES ARE ENFORCED BY THE EMPLOYER. ABATEMENT DOCUMENTATION & VERIFICATION REQUIREDTION
