were causing or likely to cause death or serious physical harm to

were exposed to a struck-by and crushing hazard:
a.On or about July 10, 2008, the safety pin was not inserted on the "Grout
Hog"
mortar bucket when it was elevated by the forklift, exposing employees
working
below to the hazard of being struck or crushed by the falling mortar
bucket.
Among other methods, one feasible and acceptable abatement method to
correct
the hazard is to ensure the safety pin is in place before moving the
forklift and
elevating the mortar bucket.
