Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of rotating parts on the horizontal engine lathe: (a)On or about Feb 11, 2008, an employee making adjustments on a Niles Machine Lathe, got his sleeve caught on a journal that was welded on the end of a rotating barrel as the result of not following safe work practices. Several feasible and acceptable methods to correct this hazard, in accordance with widely accepted industry practices, such as are found in "Safety Requirements for Manual Turning Machines with or without Automatic Control," published by the American National Standards Institute (ANSI B11.6-2001) includes: (1) Performing a risk assessment of the risks associated with operating the lathes and instituting protective measures in response to the assessment; (2) Developing safe work practices that include not allowing employees to wear loose clothing that is capable of getting entangled in rotating parts; (3) Training lathe operators in the hazards associated with the lathes, including, but not limited to, how to properly install and adjust the rollers.
