Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by a fracture (frac) valve and high pressure flow line due to failure to adequately secure the flow line: a) Mercer County, Delaware Twp, McGhee #1 Well - On or about June 16, 2008 the well casing failed during a blow down procedure when the flow line was not secured to prevent movement. The discharge end of the flow back line had not been secured to the pipe rack. ABATEMENT REFERENCE: Reference American Petroleum Institute (API) RP 54 Section 12.4.3n
