Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to elevators which were not equipped with interlocks resulting in a fall and/or crushing hazard. a)Gindy's Tire Warehouse, Inc. - On or about, 2/18/09, a freight elevator shaft was not equipped with interlocks to prevent personnel from accessing the area on an open shaft. A potential fall of up to 4 stories and a crushing hazard exist. ABATEMENT NOTE: Among other methods, feasible and acceptable methods to correct the conditions are to comply with applicable requirements notes in ANSI/ASME -2005 Section 2.6.1 (b) and 2.6.2 (a) and (b).

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were potentially exposed to a freight elevator which had not been inspected for safety: a)Gindy's Tire Warehouse, Inc. - On or about, 2/18/09, a freight elevator and its components had not had a periodic or annual inspection exposing personnel to potential fall and crushing hazards and broken bones injuries. ABATEMENT NOTE: Among other methods, feasible and acceptable methods to correct the conditions are to comply with applicable requirements notes in ANSI/ASME A17.1 - 2004 Section 8.10 and 8.11 and ASME A17.3-2008 Section 1.5 (a)
