General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to defective and damaged grinding wheels: a) Maintenance Garage/Six Inch Bench Mounted Grinder: The machine's grinding wheel was worn, rounded, and contained grooves/furrows. The grooves/furrows were present down the centerline of the wheel and on the side of the wheel. The condition potentially exposed personnel to blunt force trauma injuries/penetration wounds from the un contained fragmentation of the grinding wheel. ABATEMENT NOTE: Among other methods, feasible and acceptable means to correct the condition include: 1. Comply with National Standards Institute, ANSI B7-2000 Safety requirements for the use, Care and Protection of the Abrasive Wheels. 2. Develop program for frequent & regular inspection program for grinding machinery. 3. Inspect and ring test the grinding wheel. 4. Redress the grinding wheel if it is still within tolerances. OR 5. Install a new grinding wheel.
