Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a potential fall hazard associated with lifting personnel in an unstable aerial lift: (a)Ballast Bob's Lighting Service, Inc. & 1410 Grand Ave, Glenwood Springs, CO 81601: The Bil-Jax model # XLB-4232, hydraulic boom lift, missing outrigger pads, was used to lift employees approximately 15 feet to perform maintenance on a sign outside the City Market supermarket. This condition exposed the employee to a fall hazard of approximately 10 feet due to the instability of the lift.. Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow manufacturer's requirements associated with general safety precautions before operating the XLB-4232: 1-2 Before Operation Always inspect the equipment for damaged or worn parts. Check for cracked welds, hydraulic leaks, damaged wiring, loose wire connectors, damaged outriggers, low tire pressure, uneven tire wear, or tire damage. Also check for any improper operation. NEVER operate equipment if damaged in any way. Improperly operating equipment must be repaired before using. Abatement Note: Abatement certification is required for this item (see enclosed "Sample Abatement-Certification Letter").
