Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazard: (a) Shawn's Sparkling Squeegee Services at 1020 15th St, Denver, CO 80202: On June 13, 2008, an employee was exposed to a fall hazard and fell approximately 12 feet during window washing activities from a boatswain chair scaffolding. The employer failed to provid adequate training in order that the employee was competent, could inspect the condition of, and safely use the required equipment. Abatement Note: Among other methods, one feasible and acceptable means of abatement is to create and provide employees with a training program, inspection information and procedures and the proper equipment as required by the American National Standard/International Window Cleaning Association Window Cleaning Safety ANSI/IWCA I-14.1-2001. Abatement Note: Abatement Certification and documentation are required for this item (see enclosed "Sample Abatement-Certification Letter").-Certification
