Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of burns and bodily injury from hot steam from leaking or ruptured pressure- retaining equipment: (a)Reynolds Polymer Technology, Inc. & 607 Hollingsworth St., Grand Junction, CO 81505: On and before August 25, 2008, the employer did not ensure annual inspections were completed on the Parker boiler, serial # NB34146. The condition exposed employees to the hazard of possible burns and injuries caused by fragments due to possible leakage, rupture, or failure. Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard is to establish an operating, inspection and maintenance program for boilers as stipulated in the Colorado Boiler and Pressure Vessel Rule 7 CCR 1101-5, Effective July 31, 2006. Abatement Note: Abatement certification is required for this item (see enclosed "Sample Abatement Certification Letter").
