(a)Reynolds Polymer Technology, Inc. & 607 Hollingsworth St., Grand
Junction, CO 81505: On and before August 25, 2008, the employer did not
ensure annual inspections were completed on the Parker boiler, serial #
NB34146. The condition exposed employees to the hazard of possible burns
and injuries caused by fragments due to possible leakage, rupture, or
failure.
Abatement Note: Among other methods, one feasible and acceptable abatement
method to
correct this hazard is to establish an operating, inspection and
maintenance program for
boilers as stipulated in the Colorado Boiler and Pressure Vessel Rule 7
CCR 1101-5,
Effective July 31, 2006.
