furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to a potential fall hazard associated with lifting
personnel on a high-
lift motorized hand truck:
(a)Precision Home Builders, Inc. & 200 River Woods Road, Woody Creek,
CO 81656: On and before September 22, 2008, the employer did not ensure
that employees were trained in the safe operation of powered industrial
trucks,
in that an operator was not trained to use the Lull Rough Terrain Forklift,
model# 844C-42, serial # 00W21P22-2843. The employer did not develop and
implement an operator training and evaluation program. This condition
exposed the employee to the hazard of improper forklift operation.
Abatement Note: Among other methods, one feasible and acceptable abatement
method to
correct this hazard is to provide a training program which complies with
the requirements of
Section 5.17.4 of ASME B56.6-2002, The American Society of Mechanical
Engineers, An
American National Standard, "Safety Standard for Rough Terrain Forklift
Trucks."
