1) One (1) Sealtec employee failed to follow HESS' written policy for
signing in
and out at the refinery's main gate entrance.
2) Two (2) Sealtec employees failed to follow HESS' written policy for
signing
in and out at the control room before proceeding out into the unit to
perform a
repair.
3) Sealtec employees entered job site and performed work without following
HESS' written procedure for obtaining and signing a work permit at every
unit.
Condition occurred on or about 11/12/08.
