which were free from recognized hazards that were causing or likely to
cause death or serious
physical harm to employees:
In that an employee was exposed to a fall hazard from a "ropak" re-usable
container that was
used as an elevated work platform.  The container was placed on the forks
of a forklift and
raised to the height of 20 feet with an employee in it.  The container,
which was not attached
to the forks, shifted and the employee in the container fell to the
ground.  The manufacturer of
the container designed and intended it to be used to store and ship
products not to be used as an
elevated work platform.
ABATEMENT VERIFICATION REQUIRED
Among other methods, one feasible and acceptable abatement method to
correct this hazard
would be to use a scissors lift.
