Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer does not
furnish employment or a place of employment which are free from recognized
hazards of
inoperable emergency stops increasing the risk of being caught in
unguarded, in-going nip points
that are causing or likely to cause death or serious physical harm to
employees:
The employer does not ensure that emergency stops are operational.  This
violation was
observed on or about April 20, 2009, where the emergency stop button
located next to the drum
drive wheel machine at Station 22, Line 622 was inoperable.  Employees
operating the drum
drive were exposed to being caught in the in-going nip point between the
rotating drive drum
wheel and the pipe being rotated.
Among other methods, one feasible and acceptable means to abate this
hazard is to inspect all
emergency stops to ensure that the mechanisms are functional, without
deficiencies, in
accordance with manufacturer's design and recommendations at all locations
where machine
operators and others would be working and exposed to moving parts.
Pursuant to 29 CFR 1903.19(d), within ten (10) calendar days of the date
of this citation, the
employer must provide documentation which demonstrates the steps being
taken to ensure that
all emergency stops are being inspected and maintained.
