"Section 5(a)(1) of the Act:  The employer did not furnish to of its
employees a place of
employment which was free from recognized hazards that cause or are likely
to cause death or
serious physical harm to its employees.
This violation was last documented on December 3, 2009, when employees who
weren't tied-off
were exposed to sliding up to 260 feet down the dome roof, and striking a
parapet wall with
metal flashing before falling into gutter system.
Among other feasible abatement methods, the employer must ensure that
employees are
protected from sliding down the dome roof by the use of personal fall
arrest systems provided
at the worksite.
Pursuant to 29 CFR 1903.19, the employer must submit documentation that
this violation has
been abated within ten (10) days of issuance"
