Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: The employer did not perform formal audits of their lockout /tagout energy control procedures. This violation was most recently observed on March 3, 2009 aboard the dredge barge "Everitt Fisher" where the employer failed to conduct a continual auditing plan on at least an annual basis, whereby the audits are designed to discover any deviations or inadequacies in the procedures that may be observed during the audit inspection/audit and later corrected. ABATEMENT NOTE: Among other procedures, one feasible and acceptable abatement method to correct the hazard is for the employer to comply with the American National Standard Institute (ANSI) Safety Standards for Control of Hazardous Energy-Lockout- Tagout and Alternative Methods, ANSI/ASSE Z244.1-2003, Section 5.6.2. Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of being drawn in by ingoing nip points on rotating parts. (1)The employer did not ensure that employees were protected against the hazards associated with rotating parts when a base mounted drum hoist did have any type of machine guarding designed to prevent being drawn in by the ingoing nip points of the drum's wire rope. This violation was most recently observed on March 3, 2009 aboard the dredging barge "Everett Fisher" where the employer failed to guard the base mounted drum hoist (draw works) thereby exposing employees to the hazards of being caught and drawn into the ship's draw works. ABATEMENT NOTE: Among other procedures, one feasible and acceptable abatement method to correct the hazard is for the employer to follow the Lockout-Tagout procedures detailed in their written safety and health program, or come into compliance with the American National Standard Institute (ANSI) Safety Standards for Base Mounted Drum Hoists, ANSI/ASME B30.7-2001, Section 7-1.2.5 Disclaimers:You are not limited to the abatement methods described above.
