At this location, on or about April 29, 2008, and at times prior thereto,

Demag CC2800 cranes (s.n. 62162, 62156, 62143, and 62155) were utilized
in a lift of the upper raft float of the ATP/Bluewater Min Doc.  The
employer
did not ensure the cranes with deficiencies, such as, but not limited to,
the
following, were corrected prior to use.
a.Dry (unlubricated) load line and counterweight lines. (Demag cranes
s.n. 62156, 62155, 62162, 62143)
b.Inoperative Anti-two-blocking device. (Demag cranes s.n. 62143)
c.Inoperative wind speed indicator. (Demag cranes s.n. 62155, 62162,
62143)
d.Facility mechanics assigned to address and repair crane malfunctions
were not adequately trained for the complexities of the Demag CC2800
cranes.
e.Cranes involved in critical lifts were not certified as in good operating
condition prior to the lift.
One feasible means of abatement would be to maintain and repair the cranes
in
accordance with ANSI B30.5-2007 (Mobile and Locomotive Cranes) and the
manufacturer's Operations Manual.
