exposed
to serious fall hazards in that:
a.Employees were not instructed in the proper use, inspection, and
maintenance of tools and equipment.
b.Employees did not have available at least two means of fall protection
while working aloft.

with
paragraph 4.1.2 and paragraph 9.1.3 of ANSI 133.1-2000 Pruning, Repairing,
Maintaining, and Removing Trees, and Cutting Brush-Safety Requirements.
