placed in service:
a) On August 5,2009 and at times prior thereto at the C & S Wholesale
Grocers facility
in Newburgh, NY; the operator of the Battery Power Changer was able to
operate the
Changer while the suction cups were deployed without engaging the
manufactures safety
over-ride button.
ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED FOR
THIS ITEM
Feasible means of abatement for this would be:
1. Conduct daily inspections of the equipment by the operator as stated in
the
owners manual with a written checklist. (Checklist is available in the
owners
manual)
2. Development of preventative maintenance program that would address all
equipment needs including but not limited to checking of safety devices for
adjusting. (Checklist available in owners manual)
3. Adjustment of the devices limit switch.
Note to employer - All safety devices and functions for the equipment is
listed in the
operators manual.
