were causing or likely to cause death or serious physical harm to
employees in that:
a) Maiden Lane of Albany, Inc. 99 Washington Ave. Albany, NY- On or about
9/11/09-
Employee received crushing injuries when caught between a Humphrey Company
manlift
head pulley, mid-floor moveable overhead safety cone and ceiling of the
parking garage.
Feasible means of abatement include: 1.) Replacement of a red warning
light used to signal
employees to get off below the top floor. 2.) Replacement of an overhead
safety bar with
mercury switch. 3.) Replacement of an intrack electronic safety at the
(9") nine inch upside
position on the rear of the manlift belt on the 6th floor. 4.) Removal of
an overhead safety cone
installed on the manlift on the 6th floor. 5.) Adherence to Humphreys
Company Manlift
manufactures inspection, mechanical and safety specifications for the
manlift.
ABATEMENT DOCUMENTATION IS REQUIRED FOR THIS ITEM
