Section 5(a)(1) of the Occupational Safety & Health Act of 1970:  The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees, in
that, employees were
exposed to the hazards a rigging failure, and of being struck by falling
loads being hoisted using
equipment where the manufacturer and ANSI B30.16 prohibits lifting people,
or loads over
people.
Jobsite:
The employer used two Jet l-90 2-ton chain hoists rigged to the load line
of a mobile
truck crane in order to hoist a 5,900 pound safe placed on their flying
dock suspended
platform. On  June 07, 2008, the suspended 5,900 pound safe fell off the
suspended
platform to the ground below while employees were attempting to move the
safe from
the suspended platform through a fourth floor window opening.
A feasible and useful method of abatement to correct this hazard, among
other methods,
is to ensure that proper rigging is used on all four points of the
suspended lifting
platform (flying dock), and the use of a winch or similar device, located
inside the
building to pull the load (safe) into the building.

were exposed to the hazards associated with the moving and rigging of
loads from a crane
suspended platform:
Jobsite:
The employer did not provide an adequate stable base to support the
suspended
platform
(flying dock), and/or secure the suspended platform to the building while
moving the safe
into the 4th floor window opening.
One feasible method of abatement to correct this hazard among other
methods is the
construction of a support platform, inside the building at the window
opening that has
the load capacity for the intended load being put on it.  In addition,
securing of the
platform to the building with a clamping or similar device to ensure the
platform does
not pull away during the unloading process.g
