General-duty citation text
Section 5(a)(1) of the Occupational Safety & Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to the hazards of being struck by falling loads being hoisted using equipment loaded in excess of its safe working load. Jobsite: 8th floor roof, street side. The horizontal 1/4 inch wire rope and associated fittings used to hoist a relief angle were not capable of supporting the load where an eye bolt snapped and the load fell. A serious injury resulted when the falling load struck a worker, knocking him off of a 6' tall scaffold, and onto the concrete floor below.
