On or about Sep 10, 2008, at the Cedar Creek Relief Bridge of rural road
FM 1836 near
Kaufman, TX, employees delivering heavy equipment to a worksite were
exposed to death
and serious injuries when the bridge they were traveling on collapsed.
Among other methods, feasible and acceptable abatement methods to correct
this hazard
include, but are not limited to:
1.  Develop and implement procedures to ensure that safety elements are
assessed to make
adjustments and/or additional restrictions to posted road-bridge weight
limitations due to
stationary materials, concrete barriers, and equipment being place on the
bridge.
2.  Require employees use an alternate route for delivery of heavy
equipment to the
construction work site.
