Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that on or
about April 11, 2009, and at times prior thereto at a workplace located at
3838 Oak Lawn
Avenue, Dallas, Texas; at least two employees were exposed to a struck-by
hazard
from "L"
Buck Shipping skids loaded with 1000-1500 pounds of glass panels that were
not secured to
the forks and/or lifting carriage of a Skytrak 6042 forklift during
transport.
Among other feasible methods, one feasible and acceptable means of
abatement includes but
is not limited to:
1.   Follow the manufacturer's requirement that when picking a load move
the forks inward
or outward on the fork shaft so that they are aligned with openings in the
pallet.  Tilt the
carriage back and extend the boom slowly so the forks slide into the
openings in the pallet.
Raise the boom so the load is lifted;
2.  Follow the USAS B56.1-1969 ANSI Safety Standard for Powered Industrial
Trucks
requiring to place load engaging means under the load as far as possible
and carefully tilt the
mast backward stabilize the load.
3.  Follow the ASME B56.6-1992 Safety Standard for Powered Industrial
Trucks requiring to
completely engage the load with the load-engaging means.  Fork length
should be at least
two-thirds of the load length.  Where tilt is provided, carefully tilt the
load backward to
stabilize the load.lize
