Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushed-by hazards: On or about August 10, 2009, and at times prior thereto, at a workplace located at 235 Valtie Davis Rd.,Combine, TX 75159; the employer, by using a tied knot as a wire rope termination at the end of the truck-mounted winch and derrick load line, exposed at least two (2) employees to struck-by and crushed-by hazards. AMONG OTHER METHODS, AT LEAST ONE FEASIBLE AND ACCEPTABLE MEANS TO ABATE THESE HAZARDS INCLUDES, BUT IS NOT LIMITED TO: 1. Comply with the Wire Rope Users Manual, third edition, published by the Wire Rope Technical Board, pages 33-44.
