that were causing or likely to cause death or serious physical harm to

exposed to the hazard of falling off or being thrown off of powered
industrial truck in the event of tipover:
a) shop - on March 25, 2008, an employee was observed riding on a
Caterpillar powered
industrial truck while traveling toward the shop without using the
seatbelt provided on the
truck.
ABATEMENT NOTE:  One feasible method, among other methods, to reduce this
hazard is to use the
seatbelt provided on the lift.
