Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards which were likely to cause death or serious physical harm to employees in that, employees were exposed to hazards associated with an ALM Model 7002A automotive lift being used in an inappropriate manner to hoist/lift vehicles: a) On or about May 21, 2008 and at times prior, employees were exposed to injury in that, an ALM model 7002A automotive lift was used in an inappropriate manner as follows: 1. The steel adapter pads had a flat, smooth surface that is not compatible with mating surfaces/lifting points on vehicles on which they were used, for example, rounded leaf spring mounts. As a result, the vehicles were not hoisted in a secure manner and were subject to sliding/falling off of the lift. 2. Employees operating the lift did not have the experience and training necessary to safely operate it. 3. Incorrect extenders were used to lift a vehicle during the inspection. 4. This lift and other lifts were not adequately inspected to identify defects, such as but not necessarily limited to, the adapter pads that were in use. Among other methods, several feasible and acceptable methods to correct this hazard are to follow the manufacturer guidelines for safe operation, training, maintenance, and inspection found in the operator's manual for the ALM model 7002A lift. Another method to correct the hazard would be to implement the requirements of the American National Standard- Standard for Automotive Lifts-Safety Requirements for Operation, Inspection, and Maintenance (ANSI/ALI ALOIM-2000) as well as implement the guidelines found in Lifting it Right- A Safety Manual from the Automotive Lift Institute.
